The Secure Online System below will walk you through making your first appointment with us and is easy to do. You will enter all information through this secure online site including signing the membership form. After your first appointment you can sign into the system with your email and password to schedule appointments.   Private Membership Associations offer you True Medical Freedom and Complete Privacy. 

STEPS;

1. Select your appointments from the cloud based form below. 

2. Read and Sign Online the Membership Agreement (There is NO membership fee.  Simply read and sign the membership agreement one time only.  We are a PRIVATE health clinic therefore we are not open to the public but only see members and are accepting new members at this time.)

3. Finalize your appointment by entering name, email, phone and credit card to reserve your appointment. When registering in our secure system, your card is not charged at time of scheduling giving you option to pay with card at time of service or choose to pay with cash (significant savings). 

4. You will receive email confirmation of your appointment and reminders and an online intake form to complete prior your appointment.
